In depth: From Decision 24/7: "A large movable stone has been placed at the entrance to a drain in a bunker to prevent sand from washing into the drain. ... The stone/lid/grating/grid when used in this manner is an ​obstruction. If it is readily movable, it is a movable obstruction unless the Committee deems it immovable."
Note that an artificial drain in a bunker is an immovable obstruction (Definitions; Obstructions). 
 
Suggested wording. The title and the penalty may be removed when this Rule is placed together with other local rules under a specific headline. The last redcoloured sentence may be removed.

Drain in Bunker Covered by Lid/Stone
The stone/wooden lid/concrete lid
which has been placed at the entrance to the drain in the bunker on hole ... is an immovable obstruction. The player may either take relief without penalty in the bunker under Rule 24-2b(ii) (a) or with one stroke penalty outside the bunker under Rule 24-2b(ii)(b). Penalty: Match play - Loss of hole; Stroke play - Two strokes.
